Painkiller: Mutilator is a gameplay mod for Doom that brings the weapons and monsters of Painkiller to any Doom map. The mod features 3D weapons with alternate fire modes, hordes of familiar monsters, soul collection for a demon mode, as well as headshots, gibs, and a grappling hook.

Painkiller: Mutilator goes far beyond an ordinary weapon pack — it's a deep, high-end conversion that turns classic Doom into a true Painkiller experience. The mod takes a fundamentally different approach: instead of the usual "Doom with someone else's guns" formula, you get a full-fledged "Painkiller on any Doom map."
At the start, the player is given a choice between three characters, each with their own arsenal:
Daniel Garner: The protagonist of the original Painkiller. He starts the playthrough with the classic spinning blade and the familiar arsenal from the first game and Hell & Damnation.
Belial: The hero from Painkiller: Overdose. He has his own completely distinct loadout — instead of the usual blade he wields a mystical Cube, and his standard guns are replaced by the arsenal from Painkiller: Overdose.
Doomguy: The classic Doom hero. He uses his traditional weapon set (from the pistol to the BFG9000), but in the mod all his guns have been turned into high-quality 3D models that match the overall visual style.
All the weapons in the mod are executed with incredible visual quality: full, smoothly animated 3D weapon models whose detail is genuinely impressive on the GZDoom engine.
The mod makes gameplay as brutal and chaotic as possible, helped along by a grappling hook for fast movement across the level and a flashlight for lighting up the map's darker areas. A full headshot and finishing-move system is integrated, along with unique body dismemberment models, plentiful blood, and elaborate particle effects. Defeated enemies drop souls. By collecting them, you can enter a devastating demon mode, just like in the original game.
The mod replaces Doom's standard demons with 3D monsters from the Painkiller universe (with the addition of some enemies from Killing Floor 2). The visuals are backed by atmospheric sound design: the ambience of the original Doom sits alongside weapon sounds from Painkiller and audio effects from such iconic games as Resident Evil and Half-Life.
Painkiller: Mutilator is a magnificent, highly polished mod that delivers pure delight. It's a perfect fit both for Painkiller fans missing the atmosphere of the original and for Doom lovers looking to refresh their playthrough of classic levels or megawads.
How to install a mod Painkiller: Mutilator
To run the mod, you'll need the GZDoom engine, version no newer than 4.11.3, and doom.wad, doom2.wad, or one of the FreeDoom IWADs as your IWAD.
Make sure doom.wad / doom2.wad or freedoom1.wad / freedoom2.wad is present in the GZDoom folder
Move the mod's .pk3 files into the GZDoom folder
Drag and drop the .pk3 files onto gzdoom.exe
Play!
You can also create a Launcher.bat. This is useful for launching more complex mod builds where the loading order of mods is important, or to launch the game via a shortcut.
How to create a launcher for running mods based on GZDoom
Create a text file in the GZDoom folder, you can name it Launcher. The text can look like this, depending on which files you're using:
gzdoom.exe -iwad freedoom2.wad -file "brutalv22test4.pk3"-iwad must always be first, and the number of files after -file can be unlimited, just write them in quotes separated by spaces, like this:
gzdoom.exe -iwad doom2.wad -file "addon1.pk3" "addon2.pk3" "addon3.pk3" "addon4.pk3"Change its format from .txt to .bat by simply renaming it. Now you can launch your builds with a double-click on this launcher.
